For new cannabis patients-

The doctors in the Health Department in Santa Fe, certify patients to the NMMCP based on medical records (also referred to as a progress note). Patients are required to present a medical record, which will be attached to the NMMCP application. Your doctor can fax your medical record to this office, however the majority of patients bring their medical record with them to the consultation. There is confusion over what exactly a medical record is. It is a record of the interaction between doctor and patient, and will list a patient's complaints, past history, physical exam, diagnosis, and plan including medications. Often patients bring in tests results, or MRIs or disability statements or SSI info, or medical records that are several years old. These are not required. 

For patients already in the NMMCP-

If I am the certifying physician on a past application then I have your records on file, and I know your case and I can recommend re-certification. If you are re-certifying and are a new patient to me then you have to present medical records. The diagnosis of PTSD requires at least a yearly evaluation. At this time Dr. Love is including a PTSD evaluation along with the application with no extra charge.
